Update index.md to link to ADR-004 #70

ADR-004 was merged, need to link to it in index.md

I'm not entirely sure we should have separate issues for these instead of making it a checklist item in the original issue. Tiny issues create noise and it doesn't tie in the original issue which would be helpful for history. I imagine it makes managing your work easier though. Something to discuss.
