Create a prompt for an LLM agent to produce a form.

Tasks:

[x] Create the prompt.
[x] Tests and analytics.

Acceptance Criteria:

It should produce a JSON.
The fields of that JSON should always be same.
The content of the label should not be altered in the resulting form.
