Check that the form corresponds to an expected format.

Tasks:

[ ] Define a base model for the form.
[ ] Use the base model in the tests.

Acceptance Criteria:

All the tests should pass.
The base model must correspond to the prompt.
The router can only return a JSON corresponding to the base model.
