[BUG]Multiplying 'Toggle Focus' Options #109

My right click menu has over 50 duplicates of the 'Toggle Focus' option, they've been slowly multiplying. Only the last one works, the rest do nothing. foldernote_togglefocus_bug

I encountered the same issue where an unnecessary button is added every time I switch between workspace layouts.

I am also having repeated entries, realized at Obsidian v1.5.8
