When skipping a table, allow a note to say why #91

I would like to see this change implemented:

From chatting to NZ, this function was requested
Sometimes a whole dataset or table is not processed for a specific reason (e.g. researcher knows it does not map to their variables, or it has overlapping information with another table) - it would be good to give the user a place to log why they are not processing a table

Note - at the moment, you can still choose not to process a table! Just no note to record why in a log output
