Make install instructions the same as Synapse #83

Also clarify that PRs should go against the master branch

For some context, we use the gem method in Synapse so that folks can install the versions of dependencies they actually need (i.e. the etcd client library). Nerve doesn't really need it because we don't have many external dependencies, but I figure it's a good idea to install both consistently.
