Add github issue templates #115

This will help drive up the quality of github issues by including issue labels (triage, enhancement, bug) and other relevant context.

This will be necessary for example to ensure we can generate quality release notes (#113).

@seaneagan they were there, but I think @jezogwza 's patchset with them didn't merge so got overwritten with the last sync there's several projects where this fell off including AirshipUI, Charts & hostconfig-operator. SIP, ViNO & Images still had the templates as of Wednesday, but I've not checked them since.

Thanks, it's now merged, I left a follow up comment there, so I will leave this open until that is addressed:

I think we should request the treasuremap version as well. The airshipctl version could potentially be marked as optional, if using the default pinned version from the treasuremap version. But we can do this as a follow up.
