LH_FLAG_NOSAVE items should not trigger a "change" event when created #80

Currently an item with the LH_FLAG_NOSAVE flag will still fire a "change" event 
when created, even though no data is "changed" other than the default value at 
the time of creation.

Observed when loading a select-box-type object - possibly due to the system 
forcing the selected index to 0 instead of -1?
