Add a form for contact us/feedback #13

As a user of the platform, I would like to be able to contact the SKS hub team in order to provide feedback or ask a question regarding the Hub.

This would be a separate "Contact Us" page, accessible through the navigation bar, that would contain an embedded form (eg. google form or others) that would allow the user to write their name, email, subject and a message. Once submitted, the SKS hub team should receive an email notification with the users message.

The contact form (made in Google Forms) should be embedded onto the Contact Us page.

) It would be worth seeing if the description looks better typed directly on the website or on the form. The google form will notify the owner of the form via email when a response is submitted.

Change the dimensions of the iframe in order to make the form wider and avoid having two scroll bars
