Use TEI P5 for prepared XML form #45

In conjunction with/prior to #21 it makes some sense to use TEI P5 markup for the prepared XML form, rather than the P4 used now. All of the later CBETA releases use P5 markup, and it is easier to make the change before making new preparation XSLT.
