I've had issues like that due to the stepper stalling and thinking it moved when it didn't. Also depending on how well the gears mesh, there could be backlash which will throw off the position estimate. My guess is it's a combo of both.
I've been designing another rotator with magnetic rotation sensors to be able to have closed loop control instead of counting the steps and hoping it's correct. I'll post it to thingiverse/GitHub once I get around to building it.
