Make `Fatal` UI banner #21

Use this opportunity to also bring in the Fatal component.

Expected:

such an error should show on screen, to the user

Actual:

page seems to load. Error is only visible in the console.
[ ] error shows in Fatal banner (animate it :)
[ ] error itself is also fixed

Fixed the dev:online target, but Fatal still undone.

Did the error banner in index.html, outside of the app.

Like this approach. May eventually lead to the app being in its own repo (allows separation of concerns with reg. to ops and deployment; also allows pulling updates to ops/deployment, without conflicts in app code).
