This form looks good! I do think it is more user friendly than the purely markdown option.

Suggestions:

• Rename Institution Name to Institution or Repository Name
• Rename Description of Institution to Description of Institution or Repository
• Remove description key from Description of Institution field
• I wonder if we could make the first form element a markdown type in order to provide the contextual information that was included on the current template?
