Disable saving a form with 'empty' form name #1520

Currently, if a user deletes a name of a form and leaves the field blank, he can still save the form. This has further implications of not being able to 'see' the form in the assignments list, dropdowns, etc.

Flow needs to validate that the form name and survey name are not blank before saving.
