the user has selected a survey and form to base the map on

@loicsans proposed to avoid having too many dialogues (pop up windows telling people to first fill in name and/or select survey) but to highlight the input area and add a warning right underneath saying this information is required: "This field is required"
related to defining required and optional fields - can you add to the Map description - “(optional)” in both above the text input field and within
if the user types in the name and hits Enter (without having the survey/form selected) 'save' gets enabled
'save' is disabled after saving - ok - but does not get enabled after typing something to the name or description < need to hit Enter to trigger the change (for changing the survey/form or the colour the 'save' enabling works as expected)
Following the decisions shared here akvo/akvo-product-design#143 (comment) I am closing this issue as the functionality of Custom maps will not be part of Flow's feature list.
