Use annotator's username and password to log into the UMLS/UTS system.

4.1.    Let user set/use their own account for UMLS/UTS (about 8 hours)
4.1.1.  Add parameter in the PARAMETER system.
4.1.2.  Record user name and password in eHOST.sys.
4.1.3.  UTS user name and password must be encrypted. (PBE +MD5+TripleBES).
4.1.4.  Set user name and password: on the setting dialog.
4.1.5.  Set user name and password: a dialog; pop up while user name/password 
isn’t designated.
