add callout to warn about Input not being preserved upon closing tab #38

Currently if the survey tab is closed and re-opened in new tab the inputs submitted so far are not saved to the database not will they be retained.

It is unclear where streamlit stores this information hence not possible for us to retain atm
possible work around would be to ask people to create "login" such that they can reaccess the survey, their inputs even when incomplete. This however has drawbacks, for example it increases friction for survey

We need to make a decision how important fixing this issue is.

We dont' have the capacity to add a login functionality. Instead we will add a callout to warn users of this behavior
