Publish candidate dDID and attestation in a single ION transaction #44

Currently the candidate dDID doc + metadata are created, published and then Trustchain-resolved to get the DID document for attestation.

Instead we should be able to go directly from the created dDID doc + metadata to the Trustchain-resolved form, without publishing in between.

This would make the process of dDID creation simpler, quicker and cheaper.
