Release v2.1

Contributes towards #41.

This PR includes the following updates:

• Update to docs and file references to v2.1 covering all GB (#41) following refinements to SPENSER pipeline (spc-hpc-pipeline, #31):
  • Adds Scotland
  • Adds special areas: Birmingham, Liverpool, Manchester, Oxford, Oxford-Cambridge arc
  • Adds previously missing LADs to their counties:
  • Greater London (E09000001)
  • Cornwall (E06000053)
  • Dorset (E06000058 & E06000059)
  • Buckinghamshire (E06000060)
  • Leicestershire (E07000135)
  • Suffolk (E07000244 & E07000245)
  • Somerset (E07000246)
• Run pipeline with fixed random number generator seed (#54). Note output is not yet deterministic across platforms but is on a single machine.
• Fixed R script for step 1, outputs from more extensive refactor to be included in future releases (#63)
• Refined shell and R scripts for running step 2 and step 3.
• Add renv for R package environment.
• Shell script for upload to Azure.
