Sorry, but I need more information.
X-Plane, MSFS, Navigraph (if, what cycle), what airports (cannot read the ids) and what procedures are these?
The airports do not have any procedures here and the do not exist in latest Navigraph cycle.
I'm sorry, due to some resons(these airports data are under the export restrictions). I only save the key infomation. I think its enough, may be ? or I can add more details just hope you might fix the problem appear in approach, because it has no problem in real plane (about sid & star, hmm.. depend on you) sincerely, Eyderoe. I cannot see the RNAV approach here in LNM: ZUCK.dat seems to have the plain approach missing. It starts right away with a transition:
This is different to I21-Z, for example:
hmmm i guess you forget to choose (do not use any navigraph data) to ensure data 100% from xplane because my lnm ver. is 3.0.3.rc1 , there should not have many changes I guess.

this can prove that the bug cause by two procedures with same ident
You cannot have two procedures with the same ident. If needed a suffix has to be added.
