Muted notes not exported correctly #242

The current MIDI export logic only correctly handles notes with the string mute 
status, instead of determining the appropriate MIDI channel on a per string 
basis.  Logic will be needed to check for a muted string, to output to the 
correct channel and to write an artificial fret value of 0, since a muted fret 
doesn't hold a real fret value.

The import logic didn't have any problem importing affected MIDIs, but we'll 
need to make sure that it properly handles MIDIs created with the new export 
logic.
