By default, at least on my system, the mouse sensitivity is way too high, which makes playing the game more difficult.
I have tried modifying the 'mouse_sensitivity' variable in fallout2.cfg, but to no avail - there's no in-game change in sensitivity and the variable itself is reset to the value 1.000000 no matter what I had tried setting before. I have likewise tried varying the value of the 'MouseSensitivity' variable in ddraw.ini, but this led to no results as well.
This high sensitivity makes it really hard to make precise selections with the mouse, which is especially annoying in a 2D RPG like Fallout. Playability would IMO be greatly improved by properly implementing an option to modify the sensitivity.
