Separate one-time and recurring EP reward interface

The interface for recurring and one-time EP rewards is combined into one
panel right now, and not terribly intuitive (viz. the recent bug report
where someone thought that they had to turn off the recurring reward in
order to give a one-time reward).

I believe it would be easier for users if the one-time and recurring
rewards each had their own child panel, launched from their own button in
the main panel.  Another option would be a shared child panel with tabs to
select recurring or one-time rewards.

The recurring reward panel should either have a toggle button to
enable/disable the reward or two separate enable/disable buttons in a radio
group (instead of the current checkbox) as well as the existing reason,
amount and time period controls.  When the recurring reward is running, a
text area in the main panel should indicate as such, preferably with the
recurring amount (e.g. 100EP/15m)

The one-time reward panel should have the reason, amount, and an "apply" or
"reward" button.

If you need, I can try to sketch out my idea of how this would look, though
I hope the description is fairly complete.

I'm not sure how much change you are looking for in a 5.1 release, but I
wouldn't consider this very high on the priority list - once you know how
5.x works you can deal with it, and the completed wiki page will help in
this respect, but I believe this design would be easier for people to pick
up and run with without reading the wiki first (which they are wont to do).
