missing info about user relations in readme #94

This PR is adding the missing information about how the user in slack will be correctly assigned to the user of the commit. This is information is important to understand the problem when the private message or the {mention} placeholder in the message doesn't work as expected.

Recently the new section Troubleshooting was added. What do you think if it is a good idea to move your changes there?

And one little request: could you write "email" instead of "e-Mail"?
