This is now implemented via qgcomp.cox.boot. Following standard Monte Carlo estimators, this simulates from the underlying conditional hazard model using Efron's baseline hazard estimator (at joint quantiles of all exposures). The simulated outcomes are then used in a marginal structural model with the fixed joint quantiles of exposures and the simulated outcomes.
