You should be able to enter multiple comments per problem.

What's the UI look like for that?

Does a CommentSolution also store a line number?

When you grade something, you can assign multiple comments now.

However, there is no way to adequately associate a comment to a block of text 
or code.

Ideally, we'd want an easy-to-use interface where the user can select some code 
and then comment on that chunk of code.
