A similar issue was reported in #66. Can you try refreshing the document? If that doesn't work, try reinstalling the add-on. This may be an issue with the Drive platform.

I've added CodeBlocks to a document but when I go to Add-ons -> Code blocks, I only see "Help" menu. There is no "Start" there.
