I am experiencing the same issue.
I have tried to install the 1.4.2 version on VS 2010 with build 10.0.30319.1 of
R# 5 and no matter what I do, it doesn't appear in the plug-ins item of R#. I
can see the agent smith assembly in the R#5 plug-ins directory.

In fact I have just realised that this item was raised for R# 5.1 and I am
experiencing the issue I described in my comment of Jun 22 on R# 5.0. Is there
a known issue and maybe a work around?

To get you going, until an official release, just copy the Agent Smith folder
to the 5.1 directory.
For example, my 5.0 version sat at the following location
C:\Program Files (x86)\JetBrains\ReSharper\v5.0\Bin\Plugins\Agent Smith
my 5.1 version sits at this location
C:\Program Files (x86)\JetBrains\ReSharper\v5.1\Bin\Plugins\Agent Smith
Even though I've got the source and run a slightly custom version, when
updating it to 5.1 there were no code changes, so this should work for you.
