expand man page #1

I've added a small amount of description that would have saved me a good amount of time when starting with gnupg-pkcs11-scd, hopefully others would find it helpful as well:

In the DESCRIPTION section, the interaction between gnupg-pkcs11-scd and GPG has been expanded.

In the GNUPG INTEGRATION section, an example of adding keys as subkeys on an existing identity is described.

I'm a little unclear on github etiquette. So in order to incorporate these suggestions I'm to try a second pull request and close this one.
