Tell users how to report inappropriate content

What

Add guidance to our repos so users know:

• our repos are open and not monitored
• how to report inappropriate content

Why

Explicit content was recently published in the Design System GitHub repo. Anyone subscribed to the thread would have received a notification for this content. For more details, see our security-incident report.

By adding guidance, we can hopefully reduce the amount of time such content is visible in future, and limit the number of users who see it.

@36degrees For now, I've just raised a PR to update the govuk-frontend README. When we're happy with that wording, I'll update the other repos too.
