Create release notes for GOV.UK Prototype Kit v12 (breaking release).

Why

We're updating to govuk-frontend v4.0 (#1063)

~~We're removing jQuery (#482)~~

~~We're updating the step by step navigation template (#1117)~~

We're fixing a security issue with the Keypather package (#1155)

We're also enabling users to include query strings in their POST requests (#1120)

We've decided to skip v11.1.0 and move straight to v12, so I'm repurposing this issue for that. @NoraGDS, the release notes already cover 2 of the 5 changes, so we'll need to add the other 3, which are all breaking.
