Confirmation messages page - doesn't match prototype #701

  1. The prototype has been updated for this page, new title is "How should we send your confirmation messages?"

  2. Field labels should be "Email address" and "Mobile phone number" (not "Please enter your email address"). This reflects GDS user research.
