Add form fields to ask if donor data can be publicly exposed #176

The newsletter (and maybe the website?) would like to showcase donors and members with "high" membership tiers. This means that we need to ask permission. Two forms need adjustment:

  1. the Paypal donate form
  2. the membership (Join) and profile (Update) forms
