Observation.code's must be LOINC #17

From CCD_1.xml a number of them comes up not LOINC, some of them comes up as "system": "urn:oid:2.16.840.1.113883.5.4", "code": "ASSERTION"

Also consider populating Observation.category which maps nicely to CCDA sections.
