Multipage #104

This PR was first thought as a requirement for identifying the user for Konduto. Later we saw that it was not a requirement. But we thought that made sense to break the form into steps.

So what this PR is doing:

Adding formtools
Add template files to dbwrapper/templates/donation_wizard/
Setting the campaign_name as not required
