Datepicker doesn't function in add dialog #123

What steps will reproduce the problem?
1. Create a new form to load during the "add" button action. Must be a separate 
php file to be loaded.
2. On the form, have a datepicker element from jQueryUI
3. Click the element.

What is the expected output? What do you see instead?
Expected: Datepicker fires, select date.
What is seen: Nothing.

What version of the product are you using? On what operating system?

Please provide any additional information below.
