Structure of the in-take form

Here are the elements the form should contain in the given order:

Action items:

• [x] heading- "Book An Appointment"

• [x] Course name (dropdown menu with all the courses I offer and an "other" option)
• [x] mode - choice between online or in-person
• [ ] Date: set to the date of the time slot chosen (read-only)
• [ ] Time: start time is shown as the time slot chosen and end time can be chosen from a dropdown menu that shows options with half-hour differences upto 3 hours away from the start time.
• [x] Attach files (this can be images, pdfs (which can be textbook sized), documents, code, etc)
• [x] Comments (if they would like to mention anything to me)
• [x] a submit button with text "book"

• [ ] The page that opens when a time slot is selected should contain all the questions mentioned above as described.

The date and time tasks are more complicated than expected, I will make another issue to deal with the specifics of those two tasks before completing this issue.
