Added questions and other issue template. Updated the PR and issue templates a bit. Added github action to check the PR description

Updated the PR and issue templates a bit.
Added GitHub action to check the PR description
- It requires "Summary" and "Testing done" section
- Can skip this by adding OVERRIDE=PR_DESCRIPTION_CHECK_OVERRIDE in the PR description
