Remove FV fluid stuff from PlasmaOnCartGrid.
Remove Vlasov, VlasovPoisson, VmLBO and GkLBO equation objects and their kernels.
Remove VmLBO and GkLBO prim moment kernels. Some Lua remains because it's needed by BGK prim moments (not in g0).
Remove old commented code from Maxwell field.

The g2 build takes 172 seconds on my Mac now.
