Implement GK updater and add to SpitzerNu #112

This goes with PR #125 in gkylzero (https://github.com/ammarhakim/gkylzero/pull/125).

normNu and this build-from-scratch option are not robust, especially on the GPU and/or at low resolution. Improvements are needed.
