NonUniformRectCart does not work in parallel #15

It appears that parallel simulations, with or without shared memory, are not working when using NonUniformRectCart.

I'm seeing errors like

Fatal error in PMPI_Group_incl: Invalid rank, error stack:
PMPI_Group_incl(184).............: MPI_Group_incl(group=0x88000000, n=4, ranks=0x402cb4c8, new_group=0x402cb5e8) failed
MPIR_Group_check_valid_ranks(256): Duplicate ranks in rank array at index 1, has value 0 which is also the value at index 0

Do you have an input file which we can use to debug? It is probably something simple and if I have an example I can fix it
