Emulate underlines from PDF to signal precedence #2

In the original PDF, there were underlines to signal precedence.

There are probably still quite a few issues like this one in the converted chapters. Pure markdown is limited in functionality, but in the end I think that is good for a spec doc. We just have to find ways to express by simpler means some items that used special formatting in the original. Like you did it here.
