!TOC picks headers from code section when it shouldn't

I updated my README to use markdown-pp and used !TOC. This makes all uses of # to contain section number and doesn't exclude code section inside .... For example see https://github.com/satya-das/cib/commit/c83a84dc0b8190c55f6bbab9702ddd7a2a41c81b#diff-04c6e90faac2675aa89e2176d2eec7d8.

EDIT: I was using !INCLUDE instead of !INCLUDECODE. So, it is no longer a problem for me.
