Change session from string to array

[x] [Form] - if all day change to morning, afternoon
[x] [Event-view]
[x] [Case-view]
[x] Required
[x] Remove "All day" and add "Evening" and "Night"
[x] Change session form-item from radio to check
[x] Change in summary review and report (use session[0] as report if Time of Day)
[x] Alter database tables

Form: Events

[x] Remove "Clinic" from buttons
[x] Move Length of day down by one
[x] Add new category: Direct Clinical Care to server and client
[x] Remove CPD and LOCATION form fields for Direct Clinical Care
[x] Add default options for Direct Clinical Care (via import events datalist - v4)
[x] Event field - enlarge box and display starred/fav items below with opacity
[x] Alter database table
