Some changes to the layout #128

Added a home button

Some layout things still need to be fixed:

alignment of logos in the footer
alignment of brand image in navbar
remove white pixels in brand image
size of the table where patients can be added
size of plots and text labels in plots
fix (max)width of content again? (but make it bigger than 730 pixels)
