Disabling & Enabling 'shell' duplicates menu item under the 'View' menu

What steps will reproduce the problem?
1. Go to File->Preferences->Plugins
2. Deselect 'Shell' under 'Miscellaneous', then reselect it
3. Click 'Close', then open the 'View' menu

What is the expected output? What do you see instead?
No additional items should be added. You will see extra 'Shell' items

What version of the product are you using? On what operating system? Python
version?
SVN Revision 181

Please provide any additional information below.
Can be done numerous times, i.e., there's no limit on duplicating the menu item
