More `const`ification, and associated documentation updates. #102

Put in more const in various types where it made sense. Updated the documentation (where the types are copied).
Added more information about read-only types, clarified ownership (responsibility of release).
Added two overview tables for all the argument and result types.

@dkfellows When you find the time, can you please have a look and review, both the code changes (const added to a few more types), and the associated documentation ?
