Fix namespace handling for XSLT #26

HTML generated via XSL contains various namespace declaration on various
elements that are included from XSL templates. 

Need to work out the correct mechanism to use to allow matching (requires
namespaces, but outputs them), versus no namespaces (won't match, but
doesn't output them).
