Replace Description/Details labels with Action/Outcome(/Details) #196

Just a suggestion for increased GTDness.

When creating a new action it would be nice if the field label were actually 
Action rather than Description.

Similarly I'd like to have a (Desired) Outcome field which might or might not 
be the same as the Details box on the New Action page.

(Perhaps better still would be for Outcome to be the top field since that's 
what comes to mind first and the Action often only arises after analysis.)

And in a perfect world I'd be able to create more than one action for an 
outcome and have my outcomes listed under projects.
