Metric: More than 200 LOC/hr #117

Add a metric to CodeReviews for whether or not the Total Churn for all patchsets exceeded 200 LOC per hour.

This is based on the MSR paper metric.

How do we obtain the time factor in order to determine how many LOC was submitted overtime? Which time should we be looking at?

I'm thinking "date of first approval message" minus "date code review created".
